UCA Application Form (M/WBE) (PDF Format) The completed application must be notarized and submitted with other requested documentation as outlined in the application.
Re-Certification M/WBE -Annual Affidavit Update (PDF Format). This document must be completed, signed and notarized and returned to the Uniform Certification Agency along with the company's most recent tax return and most current financial statement.
